Regular Registration (February 15 – April 21, 2015)                $235.00

On-site Registration (April 22 – 24, 2015)                              $250.00

 The registration fee is payable by check or credit card (MasterCard, VISA, or American Express). If you are paying by check, click “Register by Check” on the payment method section on the registration form.

If you need an invoice sent to your organization for registration payment, contact Mary Ann Troyano at matroyano@aol.com. Training Orders or Purchase Orders will not be accepted.

If you are a speaker, moderator, review committee member, poster presenter or conference supporter, contact matroyano@aol.com with questions regarding your promotion code for registration.

Confirmations:

Online registration will be confirmed by e-mail at the time of registration.  All mailed or faxed registrations will be confirmed within two weeks of receipt.  If you do not receive a confirmation, please contact matroyano@aol.com.

Cancellations and Refunds:

If an attendee cancels for any reason, a full refund of the registration fee will be given until close of business on Friday, April 10, 2015.  After Friday, April 10, a refund of the registration fee will be given, less $75 for administrative costs

NOTE:  An attendee who does not cancel by e-mail to matroyano@aol.com in advance of the conference and does not attend the conference will be considered a “no show” and will not receive a refund for the registration fee.

In the event of an unforeseen circumstance, this event can be cancelled by the Prevent Cancer Foundation, and the Foundation is not obligated to refund any expenses that attendees may incur.

Policies and Procedures

  • This conference may be audio-taped.
  • Cell phones and other electronic devices must be turned off or placed on “silent” mode before entering a session.
  • Your picture may be taken during this conference and may be used by the Prevent Cancer Foundation for the purpose of reporting on and promoting the Dialogue for Action.

Meals listed on the agenda are covered expenses. These include:

    • Laurels Awards Luncheon (must register to attend)

  • Friday, April 24
    • Continental Breakfast
    • Morning Break

Expenses Not Covered

  • Wednesday, April 22
    • Breakfast, lunch and dinner are on your own. A cash-bar will be available at the informal reception.
  • Thursday, April 23
    • A cash-bar will be available at the evening reception
  • Friday, April 24
    • Lunch is on your own

All other expenses to be assumed by participants (hotel room, travel, parking, transportation, tips and alcoholic beverages)
